Part Time Intake Coordinator - Fridays, Saturdays & Mondays

We are looking to add a Clinical Intake Coordinator to our Hospice team! This position will be responsible for handling new patients from the time of first contact to the Nurse Admission. We’re looking for passionate individuals committed to improving the lives of hospice patients. The position will report to our Intake Manager, and collaborates with our administrative team, providers, intake and our community care partners. This role will be 2 days a week position on Friday and Saturday shifts. 

Intake Coordinator Responsibilities: 

• Building patient charts in the EMR 
• Entering med lists 
• Verifying insurance 
• Mapping patient visits 
• Scheduling new patient appointments 
• Communicating with patients and those involved with the patients care about the practice 
• Following up with referral source 
• Tracking new patients and referrals 
• Complete Hospice 101 
• Collect and review patient medical records, physician orders, and other clinical documentation 
• Educate patients, families, and caregivers about hospice services, care plans, and expectations during the admission process 

Intake Coordinator Qualifications: 

• High school diploma 
• 2 years of customer service experience in an office setting 
• Experience in Hospice required

The salary for this position is competitive and commensurate with experience. The total compensation pay range for this role in the state of Colorado typically falls between $27-31 per hour with and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
