Sun River Health is seeking an accomplished and dedicated part-time Physician to join our team, providing a blended scope of Primary Care, Medication Assisted Treatment (MAT), and Genesis services. This position is structured as 40% Genesis services and 60% Primary Care/MAT, reflecting the anticipated distribution of clinical responsibilities. The role is ideal for a physician who is passionate about delivering high-quality, patient-centered care in a community health setting while supporting integrated services for patients with complex medical and social needs.


The Physician will provide comprehensive primary care services, including preventive care, chronic disease management, acute care, wellness visits, and routine health screenings. The Physician will also support patients receiving MAT services and provide care in collaboration with the Genesis program, including services that may involve HIV care, hepatitis care, sexual health, STI management, PrEP/PEP services, and other related clinical needs, as applicable. The role requires close collaboration with Genesis program leadership, nursing, case management, behavioral health, and other members of the care team to ensure patients receive coordinated, supportive, and comprehensive care. 


Essential Duties and Responsibilities



  • Provide comprehensive primary care services, including preventive care, chronic disease management, acute care, and routine follow-up care. 

  • Manage a longitudinal outpatient patient panel, including patients with multiple co-morbidities and complex care needs. 

  • Conduct health screenings, annual exams, wellness visits, and other visits aligned with FQHC quality standards. 

  • Provide MAT services to eligible patients, including assessment, treatment planning, medication management, monitoring, education, and appropriate follow-up care.

  • Deliver Genesis-related services, which may include care for patients living with HIV, viral hepatitis, sexually transmitted infections, and other related health conditions. 

  • Support HIV prevention initiatives, including PrEP and PEP services, as applicable. 

  • Coordinate referrals and follow-up care with internal and external specialists to support continuity of care. 

  • Collaborate with Genesis program leadership, nursing, case management, behavioral health, and other care team members to provide integrated, wraparound care that addresses both medical needs and social determinants of health. 

  • Participate in programmatic initiatives related to Genesis services, quality improvement, grant requirements, and population health metrics, as appropriate.

  • Ensure timely and accurate clinical documentation, chart review, and completion of medical records in accordance with organizational standards. 

  • Maintain professional, compassionate, and culturally responsive communication with patients, families, and members of the interdisciplinary care team.


Minimum Education Requirement



  • Doctorate Degree, MD or DO


Licensure, Certification, and Registration



  • Active and valid license as an MD or DO in New York State.

  • Active DEA registration.

  • Board certification required. 


Specialized Skills and Knowledge



  • Experience in HIV care and MAT/Medication Assisted Treatment required. 

  • Demonstrated ability to work with a diverse patient population in a community health setting. 

  • Bilingual English/Spanish preferred. 

  • Strong clinical judgment, communication skills, and ability to work collaboratively as part of an interdisciplinary care team.

  • Commitment to providing compassionate, equitable, and patient-centered care to underserved and vulnerable populations.


Job Type: Part-Time at 22.5 hours per week


Salary Range: $133,414.20 - $151,736.10 annualized
