Major Tasks, Duties and Responsibilities

• Demonstrates appropriate skills required to work with the neonate, pediatric, adolescent, adult and geriatric patient.

• Collaborates with physicians and other healthcare providers in the assessment of medication regimens.

• Monitors the effectiveness and/or toxicity of prescribed medications and screens for medication-related problems, such as inappropriate drug, inappropriate dosing, drug allergies, and adverse drug events and interactions.

• Educates other members of the healthcare team to ensure safe ordering and administration of medications.

• Additional functions may include, but are not limited to, participation in hospital educational conferences, patient education, guest participation in hospital committees, participation in Continuous Quality Improvement measures and development of programs in other areas of professional interest.

• Assists in orientation and coordination of pharmacy student’s educational experience.
