Physical Therapist (PT) – Home Health | Per Diem

We are growing and seeking an experienced, compassionate Physical Therapist (PT) to join our team on a per diem basis in the Inland Empire area!

Key Responsibilities

• Provide physical therapy services according to a physician’s plan of care.
• Assess and evaluate patient’s therapeutic/rehabilitative/functional status and participate in developing the total plan of care.
• Evaluate home environment for safety and accessibility.
• Assess muscle strength, mobility, gait, and range of motion.
• Direct physical therapy treatment and instruct patients and caregivers in the use and care of therapeutic appliances.
• Train patients in the use of prosthetic devices.
• Prepare and submit clinical notes and progress summaries based on goal attainment.
• Participate in discharge planning and interdisciplinary case conferences.
• Provide physical therapy consultation to families/caregivers and in-service education programs for staff.
• Supervise Physical Therapy Assistants in accordance with policy and state regulations.

Qualifications

• Active California Physical Therapy License
• Minimum 2 years of physical therapy experience (home health experience preferred)
• Current CPR certification
• Reliable transportation and valid driver’s license with insurance
• Strong verbal, written communication, and organizational skills
