What You’ll Need
Master's Degree in Social Work is required
3 years of Social Work experience in a hospital/medical environment is highly preferred
Case Management / Social Work experience in a hospital setting is highly preferred
Case Manager / Social Work experience in a post-acute care facility helpful
In addition to the qualifications, a successful Care Transition Manager, Social Worker will model the factors listed below:
Working knowledge of medical necessity criteria preferred
Knowledge of Microsoft Outlook and Office (Word, Excel)
Customer service skills
Ability to engage in complex clinical decision-making
Strong oral and written communication skills
Strong commitment to interdisciplinary collaboration
Critical thinking, analysis and conflict resolution skills
Psychosocial and crisis intervention skills
Ability to prioritize and meet deadlines
