Social Worker - Per Diem - Home Health

Job Summary:

The Social Worker is a collaborative member of the Homecare and Hospice Team. Functions under the direction of the physician to provide short-term therapy to patients and/or families/representatives/caregivers with emotional, social, and environmental issues related to or impacting the patient’s illness or recovery. Assists the patient and/or families/representatives/caregivers in planning for and securing adequate community resources for patient support. Provides long-range planning, referral and linkage assistance. Participates in Homecare Case Conferencing.

Qualifications:

Education: Graduate (Master's Degree, JD or PH.D.)

Years of Experience: Minimum of 2 years experience

Licensure: State of Maine Clinical Social Worker License

Must have valid State of Maine Driver’s license and proof of auto insurance
