Position Summary: Provides patient transportation services for individuals admitted to the Swing Bed Program, including transport to and from follow-up medical appointments, under the general direction of the Director of Case Management and the Case Management team. Participates in outreach efforts to promote Swing Bed services, including contacting patients as needed to support continuity of care and ensure awareness of available services. Assists in recreational activities for Swing Bed patients throughout their skilled stay. Assists in the administration of patient satisfaction surveys to skilled patients and/or designated caregivers prior to discharge from the Swing Bed Program. Actively participates in interdisciplinary team (IDT) meetings and collaborates with the Swing Bed Coordinator to address patient needs and ensure continuity of care. Additionally, cross-trained to support operations within the division as needed.

EDUCATIONAL/EXPERIENCE REQUIREMENTS: Completion of Certified Nurse’s Aide training course required.

EXPERIENCE REQUIREMENTS:

Six months’ experience in a hospital or long-term care setting is preferred.

LICENSURE / CERTIFICATION REQUIREMENTS:

• Current CNA certification required. 


• Current CPR certification required.


• Must have a valid driver's license in good standing.
