North Canberra Hospital (NCH) is a 270 public hospital located in Canberra’s expanding northside. North Canberra Hospital operates a 24 hour Emergency Department. Inpatient services include general medicine, surgery, maternity, mental health and critical care.Other service modalities include day surgery, specialist outpatient clinics, Hospital in the Home and the Geriatric Rapid Acute Care Evaluation service that reaches into aged care facilities across the new Northside Hospital will be built on the existing Calvary Hospital campus in Bruce. It will be a modern, state-of-the-art hospital for patients, visitors and its workforce and will provide more beds and increased services. Until then, it's business as usual at the NCH, and we'll keep providing high-quality care to our patients and community.About the role:Clare Holland House provides Specialist Palliative Care Services throughout the ACT including Home-based, Inpatient, and Outpatient settings along with outreach to Residential Aged Care Facilities (RACF) and North Canberra Hospital. The Inpatient Hospice, situated on the shores of Lake Burly Griffin in Barton, provides admission for end-of-life care, symptom management & optimisation and carer respite for patients diagnosed with a life-limiting illness.Palliative Care Nurse Practitioners within the PEACE teamwork from Clare Holland House, they provide a consultative service to people who have malignant or non-malignant life-limiting illness with specialist palliative care needs who are living in residential aged care facilities in the ACT by:working at an advanced level of palliative care nursing practice that includes comprehensive holistic assessment, diagnosis, therapeutic management, quality use of medicines, monitoring and evaluation of care tailored to the person’s needs.collaborating with multidisciplinary and other health care professional/service providers to ensure the residents and their family/carer remain the focus of care, and working together with GP’s and RACF staff ensuring resident’s goals of care/advance care plan are agreed upon and upheld. The Palliative Care Nurse Practitioner will continue to develop/evolve to meet the changing clinical requirements of both CHH and the Residential Aged Care Palliative Care Nurse Practitioner will be based within the Clare Holland House, within the palliative aged care (PEACE) team and will, in collaboration with palliative care consultants, general practitioners and other health professionals, clinically manage palliative care patients within aged care facilities across the territory who have complex palliative care needs. The Palliative Care Nurse Practitioner will:Act as a professional role model, advocate and resource person in the provision of age-appropriate, compassionate, culture and trauma aware, person-centred palliative careEmpower and advocate for patients to actively participate in shared decision-making by recognising their personal autonomy in planning for future care and optimising their quality of lifeFacilitate the continual development and maintenance of standards of safe practice and delivery of comprehensive and effective specialist palliative care to patients and familiesProvide leadership to promote palliative care programs, participate in research, and maintain membership of relevant professional bodies, foster collaborative relationships with service providers and key stakeholders.Actively fostering knowledge transfer in the provision of specialist palliative care with new and less experienced staff using the framework of the National Palliative Care StandardsUndertake other duties appropriate to this level of classification which contribute to the operation of the organisation.About you:CHS is committed to workforce diversity and to creating an inclusive workplace. Demonstrated extensive and recent clinical experience, including assessment of patients with complex physical, psychosocial, cultural, emotional and spiritual needs and the development of patient centred care plans informed by evidence, and consistent with the values, goals and preferences of the patient.Demonstrated advanced clinical problem solving abilities with the ability to make decisions with minimal supervision.Proven experience in providing education to nursing and other health care staff in a variety of settings e.g. in-service programs, one on one education and conference/seminar presentation.Proven ability in quality management, nursing practice review, clinical research and policy development. Able to practice within the National Palliative Care StandardsWell-developed communication and interpersonal skills working within a multidisciplinary team, including an ability to lead and mentor other nursing staff.Computer literacy and an understanding of clinical management systems, word and spreadsheets.Demonstrated commitment to and understanding of the principles of quality management, clinical research and policy development in the palliative aged care health care setting.Demonstrates understanding of, and adherence to, safety and quality standards, work, health and safety (WH&S) and the positive patient experience.
