Job Description - Manager, Home & Community Care

Job Title: Manager, Home & Community Care Locations: Niagara On The Lake, Ontario Job Type: Permanent Full-Time Workplace: Hybrid – 50% in Community (candidates must live locally) At Vyta, we simplify aging in place for seniors and their families. Powered by Bayshore HealthCare, the successful candidate will join a team focused on promoting well-being and freedom by working closely with each family to help advise, coordinate, and deliver a wide range of services to keep seniors at home as long as possible. As the new leader of one of Bayshore’s local Neighborhood Health Teams (NHTs), you will collaborate with an integrated regional team to oversee a range of in-home care services, manage client service programs, and oversee all team operations. This role is an active community position that involves a high level of hands-on care, an entrepreneurial spirit, community outreach, and building partnerships with clients and caregivers in the community. Duties & Responsibilities The responsibilities of this role include, but are not limited to:
  • Direct and monitor the delivery of all in-home services and daily functions of field staff providing access to clinical advice, support, and resources.
  • Assist with the delivery of in-home services as the on-call nurse, as needed.
  • Lead the sales process and business development, client acquisition strategy and marketing activities for your Neighborhood Health Team (NHT).
  • Build and sustain strategic and collaborative relationships with current, new and prospective partners and community organizations.
  • Assess and document community needs while developing business and funding proposals to meet emerging opportunities.
  • Develop comprehensive plans for clients and families using a holistic approach to help them achieve their aging in place goals, and continue to resolve client’s challenges by offering services, products, advice, and community resources.
  • Hire and train all NHT staff members, including training and orientation on all services, products, and pricing; supervise the day-to-day performance of the team; ensure exceptional interactions with prospective and existing clients; and provide ongoing coaching and performance management.
Candidates Must Have
  • A current and valid Registered Nurse (RN) or Registered Practical Nurse (RPN) registration with the CNO.
  • A minimum of five (5) years of recent experience, preferably in a community setting, with at least two (2) years managing others.
  • Current and valid CPR certification.
  • Experience in community work and working with community partners in a collaborative way.
  • Experience with business development and in engaging with municipal and provincial groups is strongly preferred.
  • A reliable vehicle and willingness to travel to visit clients within the community (roughly 60% in the field, vehicle allowance is provided).
  • A solid knowledge of the principles, practices, and methods of program development, implementation, and evaluation.
