Job Description - Team Lead Social Worker

The salary range for this position is CAD $47.24/Hr. - CAD $58.99/Hr.
Mandatory Vaccination

This role will work on our Road to Recovery After Care team.

 

Within the context of a patient and family-centered model of care, and in accordance with the Mission, Vision, Values, and strategic directions of Providence Health Care (PHC), promotes a safe, respectful, and civil working environment. Reporting to the Professional Practice Leader (PPL), Social Work, and working collaboratively with Leadership of the Road to Recovery Program, the Team Lead, R2R After Care, administers, oversees, and coordinates the daily operations and clinical services offered by the R2R After Care team. The Team Lead ensures the seamless delivery of mental health and substance use services for adults who are seeking support for their substance use. The Team Lead acts as a clinical resource to staff, students and the interprofessional team members and promotes interprofessional collaboration. Establishes priorities and coordinates work assignments and schedules. Identifies learning needs and interests of staff, conducts performance evaluations, and provides formal feedback. Provides educational and orientation programs and presents to staff as required. Interprets, implements, and monitors policies, procedures, and standards related to client care. Collaborates with other interdisciplinary team members and interfaces with other departments, health. and community providers to ensure timely and seamless delivery of clinical services. Provides individual counseling to clients with complex mental health and substance use, drawing on evidence-based treatments. Coaches, mentors, and supports staff and facilitates effective problem solving as needed. Supports student placements within the program. Identifies, coordinates, and promotes research and quality improvement initiatives. Participates in team and/or local committees, internal and external case conferences, and quality improvement activities. Supervises at least one (1) FTE Social Worker and more than eight (8) FTE up to and including sixteen (16) FTE total staff.

Education, Training, and Experience
Masters in Social Work. Four years (4) recent, related experience working with adults with mental health and substance use issues, including previous experience providing educational leadership and supervision within a community health care setting, or an equivalent combination of education, training, and experience. Post graduate continuing education in models in substance use and mental health. Training in Dialectical Behaviour Therapy, Motivational Interviewing and Attachment Theory and its application is required. Current full registration with the British Columbia College of Social Workers. Skills and Abilities
•Comprehensive knowledge of psychiatric diagnoses and professional practice guidelines for the treatment of various disorders.
•Demonstrated knowledge of substance use issues and related evidence-based best treatment practices, including opioid replacement therapies.
•Knowledge of psychotropic medications and side effects.
•Demonstrated therapeutic skills and the ability to apply theory and practice in the areas of case management, counseling, individual and group therapy to clients with a variety of mental health and substance use disorders.
•Demonstrated comprehensive knowledge of relevant Acts, pertinent regulations, policy, procedures and other statutory and certification requirements (such as the Mental Health Act, Adult Guardian Acts, Child, Family and Community Service Act, and Freedom of Information Act).
•Broad knowledge of attachment and self-regulation theory and strategies.
•Demonstrated ability to lead and supervise others.
•Demonstrated ability to identify learning needs, teach, coach and provide clinical information and guidance.
•Ability to educate and provide consultation to clients, families and healthcare professionals.
•Ability to develop, coordinate and deliver in-services, presentations, psycho-educational and support groups.
•Demonstrated ability to conduct comprehensive psychosocial assessments that include mental health and addictions standardized frameworks and tools.
•Ability to assess, plan, organize, implement, evaluate, and problem-solve.
•Ability to establish rapport with adults who are experiencing multiple barriers to accessing health care.
•Ability to communicate effectively with individuals, groups and other professionals within an interprofessional environment in a diverse community.
•Ability to work effectively with others, including working effectively in a multicultural environment, independently and as part of an interprofessional team and understanding the frameworks of other disciplines.
•Demonstrated ability to organize and prioritize workload in a complex environment.
•Ability to manage crisis situations as well as to provide leadership to staff in the management of client crises.
•Ability to make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other community partners.
•Proficiency in the use of personal computers, including related word processing and database management software applications.
•Physical ability to carry out the duties of the position.

1.Oversees and coordinates the daily operations and clinical services offered by Road to Recovery After Care Team and provides leadership to the staff within the program by performing duties such as supervising, coordinating work assignments, establishing priorities, and providing critical input on employee performance.
2.Assists Road to Recovery Leadership with planning implementing and evaluating service delivery for designated client areas through methods such as identifying service gaps or recommending changes to improve service delivery.
3.Develops, reviews, revises, and provides input into policies and procedures and related work practices to ensure clinical and organizational standards regarding safety, security, confidentiality, and clinical care are adhered to. Assumes accountability for ensuring organizational policies and procedures are followed, changing working practices where required. Coordinates, develops, implements, and evaluates clinical procedures, education strategies, and program communication materials. Solicits feedback from the team and reports on program activities to Road to Recovery Leadership.
4.Monitors and evaluates outcomes of therapeutic services, ensures referral, intake, and wait time information is tracked and maintained. Advises and works with the SW PPL and/or program staff to resolve issues/concerns and problem-solves complex client/family care issues such as those involving the child protection act, adult guardianship, and legal/liability exposure.
5.Acts as a clinical resource to staff, students, and other members of the interprofessional team by supporting the development of individualized care plans, demonstrating procedures, answering questions related to clinical practice issues, troubleshooting concerns and/or making necessary interventions. Provides a forum for staff communication and problem solving through staff meetings. Ensures necessary resources are available or accessible and delivers related training and orientation requirements.
6.Provides clinical social work practice to clients and families using advanced therapeutic counselling skills and by methods such as bio-psychosocial assessments, gathering pertinent social data regarding the client/family, environment, culture, living situation and financial status and interpreting the data using a variety of theoretical modalities and in accordance with professional standards and clinical policies and practices to therapeutically impact the client/family’s coping abilities.
7.Works with PPL SW to promote educational opportunities for staff and presents and/or coordinates in-service education. Responsible for program orientation to employees as required.
8.Participates in the recruitment and selection of staff as needed including initiating posting requests and participating in selection panels.
9.In conjunction with the PPL, and Research, Education and Practice Coordinator, supports the student fieldwork experience program by finding placements and providing student orientation.
10.Collaborates with interprofessional care team members regarding Quality Improvement (QI) initiatives, by developing and leading QI projects and initiatives, implementing processes, procedures, and standards upon authorization, and evaluating results. Develops assessment forms and client materials, program descriptions, audit tools and clinical guidelines. Ensures staff are aware of changes and updates once established.
11.Maintains and fosters relationships with key stakeholders and community organizations. Acts as an advocate for the goals and objectives of the service and for client/program access to community resources. Builds and enhances community relationships and communications and facilitates new opportunities and partnerships by methods such as contacting relevant programs/services, providing information regarding R2R After Care services and activities, and assisting in identifying program/service needs for communities. Follows up/reviews concerns regarding program effectiveness. Provides ongoing updates to R2R Leadership team and other interdisciplinary team members.
12.Maintains a variety of records such as workload statistics by methods such as recording information into approved formats, collating data, analyzing trends, accounting for variances, and recommending an action plan to the R2R Leadership Team and the PPL, Social Work and/or other designate.
13.Identifies relevant research opportunities and integrates the application of research findings into practice by incorporating information into policies, procedures, educational programs and materials. Participates in research initiatives and projects related to client care and/or discipline practices as requested and disseminates findings.
14.Takes leadership role in conjunction with Leader/PPL in professional practice council meetings for Social Work and advises Leader/PPL of concerns or issues. Attends and/or leads meetings, in-services, and corporate initiatives by methods such as presenting relevant materials/ topics to remain informed of departmental/ practice activities and to contribute to program initiatives.
15.Performs other related duties as assigned.
