Komfort Kare also acts as an HSE provider. We offer clients a range of behavioral support, domestic assistance, personal care, complex care, ASD and ID Support, and 24-hour health care services.

Job Role

As a Trainee Care Manager with Komfort Kare, you will be allowed to take full ownership of your area while under the guidance of a senior Care manager or Senior Operation Manager. Liaising directly with clients and support workers to ensure full coverage of schedules.

The Care Manager will support in completing the administration tasks that go along with rostering and scheduling care for their area and is an excellent opportunity for a dedicated candidate to take their first step into a management role.

The ideal candidate will be capable of managing all aspects of client schedules. They will take responsibility and accountability, possess excellent communication skills, and be dedicated to providing the highest quality of home care to those most in need.

Job Description

• Creating and updating personalized care plans for patients.
• Collaborating with patients' families, friends, and social supports in developing treatment plans.
• Identifying and recruiting high-quality healthcare providers.
• Interacting with healthcare providers to facilitate patients' treatments.
• Ensuring patients’ treatment requirements are met.
• Suggesting alternative treatment plans when patients' service requests do not meet medical necessity criteria.
• Instructing and educating patients/Clients on procedures, healthcare provider instructions, and referrals.
• Linking patients/Clients to social services programs and entitlements such as transportation assistance and translation services.
• Conducting regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes.
• Maintaining records of case management activities.
• Proficient with Healthcare Management Systems and Microsoft Office Word and Excel
• Outstanding communication skills, both written and verbal.
• Excellent organizational skills.
• Ability to travel locally to assist carers if needed.

Salary Range

• From €34,000 to €37,000 (DOE)
• If you passed your probation and have archieved all your KPI's you can get a pay rise up to €40,000!
• On Call Allowance

Schedule

• Monday To Friday - On call shifts after office hours
• Rotation on Weekends as Support Manager for On Call
• 9am to 5pm
• Based in our Dublin Office

Essential Requirments

• 3 Years of Healthcare Exprience
• 2 years of Scheduling Exprience
• Be proactive with excellent communication, negotiation, problem solving and decision making skills

What will be required from you to complete:

• Garda Vetting
• International Police Clearance (If you have lived 6 months or more outside of Ireland from the age of 16 years old)
• Patient Manual Handling - In house training
• 2 Days Induction - In house training
