Far North Community Hospice extends its care to patients, their families, and whānau living in a beautiful area of Te Tai Tokerau, spanning from Taupō Bay to Te Rerenga Wairua (Cape Reinga) and coast to coast. We cater to a resilient, vibrant community, including areas with high deprivation and rurality. Our core mission is to empower patients and whānau, alongside healthcare providers and the wider community, through the delivery of exceptional palliative care, bereavement care and education.
We are excited to introduce a new and essential role within our organisation: Specialist Palliative Care Social Worker. In this capacity, as a Registered Social Worker you will guide and support patients and their whānau throughout their end-of-life journey, utilising a whānau-ora approach that emphasizes family involvement and leadership. Your expertise will encompass an understanding of health variances for Māori, adherence to the principles of Te Tiriti o Waitangi, and a commitment to health equity. A proven track record of advocacy is essential.
As a vital member of the Patient and Whānau Support Team, your primary focus will be identifying the psychosocial needs of our patients, their support networks, and the bereaved, ensuring they receive timely and appropriate support. You will work closely with our multidisciplinary hospice team simultaneously establishing and fostering collaborative relationships with primary care practices, an aged care facility, Māori health providers, hospitals, government agencies, home-based care providers, social work services, counsellors, and other community organisations.
Our commitment to providing excellent palliative care and support to everyone, regardless of ethnicity, income, or location, is firm. This role presents a meaningful opportunity to connect with patients and their whānau, providing support tailored to their needs and in environments that hold personal significance. With a significant portion of our work occurring in the community, particularly in the homes of our patients and their whānau, this role plays a pivotal role in meeting the preferences of nearly 90% of patients in Te Tai Tokerau who wish to spend their final days at home.
