Team Leader - Case Manager

SUMMARY:
Our client is seeking dedicated and experienced case managers to join their team and manage the spending of member funds through clinical interventions and the application of managed care protocols. Role will be critical in optimising patient care and ensuring efficient resource allocation.

POSITION INFO:
Responsibilities:
Assess the medical needs of patients and provide recommendations for cost-effective care.
Facilitate patient access to appropriate healthcare services and resources.
Monitor and evaluate patient progress and treatment outcomes.
Provide support and education to patients and their families.
Document case management activities and maintain accurate records.
Collaborate with cross-functional teams to meet organisational goals and standards.

Experience:

• A minimum of 5 years of general medical and surgical nursing experience in the private sector
• ICU training and/or at least 1 year of ICU nursing experience
• At least 1 year of hospital case management experience will be advantageous

• Nursing Degree/Diploma
• Professional Registered Nursing Sister with a valid and up-to-date registration with the South African Nursing Council

• Excellent clinical assessment and decision-making skills
• Strong interpersonal and communication abilities
