Job Description - Behavioral Health Consultant

The Staff Pad has partnered with the largest community-based behavioral health and substance use treatment services organization in the state of Oregon. Our client is a Certified Community Behavioral Health Center seeking a Behavioral Health Consultant, LCSW  to join their organization. 

The Behavioral Health Consultant (BHC) works as a member of a multidisciplinary Primary Care medical team in managing the whole health of Primary Care clients. The BHC will provide consultation, collaboration between Primary Care and Specialty Behavioral Health, and point of care behavioral interventions within the Primary Care setting. The BHC will support a broad range of needs within the Primary Care patient population and will function as a core member of the team.

 

Essential Duties:

  • Provides targeted evidence-based assessments and evaluations for behavioral health issues identified within the Primary Care Setting.
  • Provides brief, point of care, solution focused interventions to Primary care clients to promote well-being and assist with implementation of treatment plans
  • Provides skill building, coaching, and education to enhance whole health
  • Provides feedback and recommendations to Primary Care team regarding consultation findings. Consultations can be both formal and informal.
  • Provide advocacy and case management services for clients
  • Maintains knowledge of community resources and refers out as appropriate
  • Collaboration with internal specialty- Outpatient Mental Health and Addictions
  • Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations. Maintain accurate and up-to-date documentation as required, associated with billing, assessment, treatment, client’s progress and other activities.
  • Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for clients and staff.
  • Provide crisis intervention services in order to minimize or eliminate periods of crisis and acute care utilization.
  • Requires some travel between clinics for various meetings and potential occasional evening work
  • Meets performance standards for productivity, documentation and adherence to metrics
  • Adhere to compliance, required laws and trainings, safety of property and staff, regulations, I.e. Mandatory abuse reporting laws and HIPPA, fire regulations
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.
  • Complete all annul employee training requirements in order to understand how to conduct ethical work, for professional development, and stay in compliance
  • Perform other duties as assigned

 

Qualifications:

  • Must be a current LCSW
  • Requires 2 years of clinical experience; primary care experience is preferred
  • Knowledge of community resources
  • Basic understanding of common medical conditions and terminology
  • Knowledge of culturally relevant and specific approaches and resources
  • Perform a mental health assessment
  • Knowledge of Trauma Informed Care
  • Knowledge of Motivational Interviewing and evidence based practices
  • Understanding of Recovery Model Understanding and Illness Management
  • Ability to Type 50 WPM
  • Knowledge of Microsoft office suite
  • Intermediate Excel knowledge and able to prepare advanced reports
  • Operate computer programs and systems at a level needed to perform the essential duties
  • Possess a valid driver’s license, three year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete an online Defensive Driving course. Employee must be approved to provide transportation utilizing agency vehicles so that residents may access the community.

Work is generally performed during standard business hours, 8:30 am to 5:00 pm, in an office and health center environment. Requires flexibility to work evenings.
