Position Title: Social Worker
Location: New York City, NY 10032
Duration: 6 Months
Work Schedule: M-F 9.00 - 5.00
Responsibilities for this position include, but are not limited to:
- The Audubon clinic serves individuals with serious mental illness, the majority of whom are Hispanic and reside in the Washington Heights neighborhood of Upper Manhattan. The social worker will carry a caseload of patients with serious mental illness and will contribute to our mission of providing recovery oriented services through case management and supportive therapies. The social worker will formulate multidisciplinary treatment plans, psycho-social assessments and progress notes. The social will be part of a multi-disciplinary team working with psychiatrists, nurses, peer specialists, occupational therapists and a team of other social workers.
