About The Role
We are seeking multiple full-time applicants with customer service or case management experience who will manage a personal portfolio of clients, providing ongoing customer support and championing our consumer-directed care approach.
This role will see you working from our vibrant Fortitude Valley office. Fully paid on-the-job training is provided, so no industry experience is required.
Duties
- Become the trusted point of contact for your personal portfolio, ensuring ongoing communication with clients, their representatives, stakeholders, and service providers
- Assist consumers to enact their care plans and budgets, ensuring needs and goals are supported under the Home Care Package framework.
- Source & manage information to support consumers in achieving their documented care outcomes.
- Update and maintain consumer records for all matters related to care management as required.

- Service-orientated with strong interpersonal and communication skills
- Strong computer skills with intermediate-level knowledge and application of MS Office suite
- Well-organised, able to prioritise, and meet competing deadlines
- Excellent attention to detail, able to problem solve and make decisions
- Enjoys working in a multi-disciplinary team
Salary & Benefits
- $60,000 - $70,000 per annum (commensurate with experience) + 10.5% super

Mandatory Requirements
- Current Federal Police Clearance (not more than 12 months old) and international Police Check if you have lived overseas in the last 10 years
