NDIS Support Coordinator

Focus Care is seeking a dynamic and passionate individual to join our Sydney team permanent, full-time in the Strathfield Head Office. In the role as a Support Coordinator at Focus Care, you will be supporting NDIS participants across Australia-wide.

Our goal is to ensure we can redefine traditional healthcare and ensure our clients and participants can live independently in their home respecting their choice, control, and dignity. About the Role:

• Develop and maintain trust and rapport with participants and their support networks
• Supporting participants to maximise their NDIS funding to ensure it adequately meets their support needs
• Building the participants, and/or informal supports’, capacity to advocate for themselves and navigate the NDIS more confidently
• Critically analysing participants’ support needs to identify gaps in their service provision, highlighting relevant referrals to other professionals
• Completing implementation and review reports to support participants maintain/increase their funded supports and comply with NDIS legislative requirements
• Manage intake of Support Coordination enquiries to drive growth alongside the National Program Manager (NDIS)

Skills/Qualifications:

• High-level of organisational, time management and planning skills
• Strong problem-solving skills
• Excellent communication and customer service skills
• Great technological literacy
• Initiative and willingness to learn in a fast-paced and evolving environment
• Allied Health qualifications (such as Social Work); or minimum Certificate IV in Allied Health/Disability/Aged Care or similar is not essential, but highly desirable
• Valid full driver’s license (own vehicle is not required)
• NDIS Worker Check and WWCC (or willingness to obtain before commencement)
• Minimum two doses of COVID-19 vaccine
