The Access Aged Care team provides co-managed primary care services to Aged Care residents across Australia.Your New RoleAs you step into this exciting onsite part-time opportunity reporting to the Nursing Regional Manager, you become an integral part of our flourishing organization in a newly created position dedicated to improving the health outcomes of our valued this key role, you will spearhead clinical management and coordination efforts, collaborating closely with residential aged-care facilities. We seek an individual with motivation and passion, capable of thriving in an autonomous role that offers both freedom and flexibility. Your base will be in aged care facilities within the Oakleigh region, and you'll frequently travel to partner homes to conduct clinical reviews and manage other responsibilities. While your primary focus will be on Oakleigh, occasional travel to other areas may be Registered or Enrolled Nurse will collaborate within a multi-disciplinary team, ensuring efficient and high-quality service delivery, backed by the support of the Nursing Regional Manager and the administration team. The successful candidate will also contribute to our care model by engaging in organic business development to expand the resident base.
