The Organisation
Nganampa Health Council (NHC) is an Aboriginal Community Controlled Health Organisation operating on the Anangu Pitjantjatjara Yankunytjatjara (APY) Lands in the far north west of South Australia. The APY Lands are home to almost 3,000 residents and the Anangu culture is strong with Pitjantjatjara/Yankunytjatjara being the first language.
NHC operates clinics 24/7 in the six main communities in the APY lands, one outreach clinic and health programs including in sexual, environmental, dental, women’s, male, children’s, eye and mental health, health worker training and immunisation.

The Role
The Hospital Liaison Officer (HLO) is a member of the NHC Hospital Liaison Team and provides services that support the facilitation of patient referrals and provision of transport, accommodation and social support for Anangu who must travel off the APY Lands for secondary or tertiary level health care. The HLO liaises with other stakeholders in the service delivery network such as NHC clinical staff and staff at the Alice Springs Hospital, Aboriginal Hostels and local transport providers. The goal of this work is to facilitate referrals and ensure that the patient journey for Anangu is well coordinated, effective and efficient.
The HLO reports to and works closely with, the Senior Hospital Liaison Officer, participates in Team meetings, and makes referrals for social work services to NHC’s Patient Support Services Social Worker.
The role is responsible for ensuring:-
• that the patient journey for Anangu is well coordinated, efficient and culturally sensitive
• accommodation and transport requirements for Anangu referred from the APY Lands for secondary/tertiary level clinical services are met
• relationships with external stakeholders in our service delivery network including Hospitals, Transportation companies and Accommodation Providers are working well
• all documentation and records are correctly entered into Communicare Clinical Information System and Patient Appointment Database
• data and reporting is maintained to monitor the Team’s outputs, costs, and service delivery gaps and trends.
Skills Required
You have experience in inter-cultural work with Indigenous Australians in remote areas or other challenging environments, together with ability to participate as a member of a small team.
You’ll also be able to demonstrate your experience in:
• Literacy and Numeracy skills required for role
• A well organised approach to work and attention to detail
• Teamwork and communication skills.
• Understanding the importance of confidentiality and demonstrates this in their practice
• Can provide a current Police check and Working with children check, or willing to obtain
• Current Australian Drivers Licence
• Experience using MS Office - excel, word, accounting software and Patient information systems
