About the role


The Social Supports Program is an opportunity for clients to attend and participate in social interactions which are conducted away from the client's home, through structured activities that are provided in a group-based environment and designed to develop, maintain and support social interaction and independent living.


The Social Supports Program Activity Workers are part of a broader team working with the individual clients to optimise their health and independence. They play a key role in monitoring the person's progress towards their goals and ensuring that the program structure meets the needs of the client. 


 




Key responsibilities 





  • Be a part of a broader team to ensure innovative and high standards of care

  • Work with the individual clients, their families/carers and external providers to optimise their health and independence. 

  • Play a key role in monitoring the person's progress towards their goals and ensuring that the program structure meets the individual needs of the client.

  • Located at 120 Settlement Road, Belmont, but maybe relocated to other Barwon Health locations depending on service delivery needs.  

  • Operate according to relevant legislation, Barwon Health policies, program and department guidelines.


 




What we’re looking for 





  • Minimum Certificate 3 qualification in relevant health field, or working towards

  • Current Victorian Driver’s Licence

  • Demonstrate highly developed interpersonal and communication skills

  • Ability to organise activities efficiently and courteously

  • Awareness of and sensitivity to people with special needs

  • Committed to creating an inclusive environment where everyone feels comfortable participating

  • Demonstrated knowledge of Aged Care System (Reforms) and Community Health framework

  • Comply with the Aged Care Code of Conduct and Statement of Rights
