Key responsibilities:

  • You will conduct assessments of the elderly indigenous clients to create a comprehensive understanding of their physical, emotional, social and cultural needs, to assist in the development of individualised care plans in a holistic approach (including client, families and other healthcare agencies).
  • You will support the client and families through the aged care processes internally and externally to ensure comprehension of agreements and support care plans.
  • You will ensure the health, safety and wellbeing of the client is prioritised, maintained and managed across all accesses to support services.
  • You will coordinate the access to care services, including medical care, home support services, transportation and social activities.
  • You will liaise and engage with indigenous community members to raise awareness about aged care services and promote healthy aging practices.

Essential 

  • Qualification in Individual Support or Aged Care (or other relevant qualification)
  • Demonstrated experience with support coordination.
  • Demonstrated knowledge and understanding of Aboriginal and Torres Strait Islander cultures preferably with established links to the local community.
  • A current Driver’s Licence 
  • Victorian Working with Children Check
  • Victorian Police Check that doesn’t disclose any serious offences, and an ability to maintain currency over the duration of the employment contract. 

 

Key Selection Criteria

  • Knowledge of and experience with local aged care support services and accessing supports, as well as coordination of supports.
  • A sound knowledge of current health issues relevant to Aboriginal & Torres Strait Islander communities 
  • Willingness and commitment to work effectively as part of a multi-disciplinary team and deliver services to the whole community.
  • Clear and effective written and oral communication skills that are readily understood by the intended audience. 
  • Computer literacy and ability to use computer applications to generate reports, and assist with basic research and collection of data.

We are a Community Controlled Organisation (Not-for-profit) that has been servicing the Aboriginal and/Torres Strait Islander community since 1994 when it was established by local Aboriginal Community Members. The purpose of MAC was and continues to be promotion of a healthy and proud community through motivation, education and cultural acknowledgement while ensuring a sense of belonging. MAC has grown to over 30 staff members and is projected to grow exponentially over the coming years. 

As MAC’s Aged Care Support Coordinator, you will report to Community Health Team Leader. You will contribute positively to the multi-disciplinary Community Health team and engage and build external relationships with local stakeholders and agencies, with the common purpose of wellbeing and healthy aging for our elders. You will harness our values of cultural proudness, empowerment, collaboration, respect, integrity and compassion in all aspects of your position within community and MAC and will have these values reciprocated.
