We are looking for an experienced and motivated Care Advisor to join our Home Care Packages Team on a permanent, full-time basis. This role is instrumental in providing support and advice to the Home Care Packages clients to assist them to continue living at home. Your responsibilities will include day to day management of a client case load, conducting assessments, care planning and provision of consumer directed services to complex care clients within budget guidelines. You will build professional relationships internally and externally and look for creative care solutions to achieve the best outcomes for clients. This role involves travelling to meet clients in the community as well as working in a busy office environment.
Essential:
Tertiary qualifications in a Social or Health Science, Registered or Enrolled Nurses with current registration.
Knowledge of Home Care Packages, My Aged Care, and Aged Care Quality Standards,
Maintain direct contact between the service provider, the client and their representative, constantly monitoring care needs and service delivery, including visiting clients in their homes
Constantly review and modify service approaches in response to changing circumstances, individual needs of the client and current relevant evidence-based care
Assist client’s transition from one service to another and from one level of care to another
Develop knowledge and networks to meet client special needs
Liaise with external agencies e.g. the clients’ GP, other health professionals, ACAT etc
Identify and report on service gaps within the program
Work within mecwacare’s policies and procedures including confidentiality and privacy guidelines
Assist with other professional duties as required in line with the general scope of case management, professional qualifications, experience and skills.
High level proficiency with the Microsoft Office suite including Word, Excel and Outlook.
Excellent presentation skills especially to community groups.
Current Victorian Driver’s License
Desirable:
Experience with brokerage models of service and principles of Consumer Directed Care.
Experience in liaising and negotiating with community service providers.
Knowledge of Carelink Plus.
Knowledge, skills and experience in delivering specialised care to different groups within the aged population.
