Reporting to the Integrated Team Care Team Leader, the purpose of this role is to work collaboratively with ITC Clients, General Practices, Aboriginal Health Services Specialist and Allied Health providers to support appropriate multidisciplinary care and services for Aboriginal people.

The position will assist to increase the capacity of GPs and other health professionals in delivering a holistic health care service to Aboriginal Clients whilst working within the guidelines of the Integrated Team Care Program across identified communities within the HealthWISE region. The position will provide delivery of ‘best practice’ care in the areas of health education, health literacy health promotion, and Chronic Disease Management.

The Care Coordinator (ITC) will be required to work closely with other members of the Aboriginal Health Access Team, as well as the broader HealthWISE Team to achieve the outcomes of this program in an effective and patient focused manner across the New England geographical region.

The Care Coordinator (ITC) is required to work both independently and as part of a team, applying their expertise to ensure the delivery of culturally appropriate and safe clinical support to clients of the service. The successful person shall apply their community knowledge, skills, competence, and training to achieve the HealthWISE goals within timeframes and approved budget. The position shall add value to all activities and services of HealthWISE, working collaboratively within a broader team environment to assist in facilitating and achieving HelathWISE’s Vision and Purpose.

HealthWISE considers that being Aboriginal is a genuine occupational qualification under s14 of the Anti-Discrimination Act 1977 (NSW).

Key Responsibilities

  • Promote cultural awareness and competence within the HealthWISE team and in interactions with consumers, stakeholders and communities.
  • Promote the safety, wellbeing and empowerment of children.
  • Comply with HealthWISE policy and procedures and Code of Conduct as well as all relevant legislation and regulatory standards.
  • Actively contribute to the growth and financial sustainability of HealthWISE through identifying new business opportunities, efficiencies, cost savings and innovative solutions.
  • Promote HealthWISE Aboriginal Health Access programs to the community.
  • Deliver appropriate care and follow-up and arrange referrals for clients, in line with GP requests that meet ITC guidelines.
  • Source specialised support and consultation where needed, facilitate access to community services and appropriate primary health care providers as required.
  • Advocate for the rights and needs of individuals and their families.
  • Support GPs in providing early intervention and preventative services within your scope of clinical practice where requested.
  • Assist with specialist clinics to ensure cultural appropriateness.
  • Obtain and record accurate health information to ensure compliance to all organisational policies, procedures, and legislative requirements.
  • Liaise with general practices and their patients around requested completion of health assessment and care plans.
  • Endeavour to build a comprehensive case history incorporating knowledge, skills and networks of individuals and communities to enable community members to manage their condition/s.
  • Participate in the development and delivery of health promotion and education strategies, as well as preventative health programs and activities, both individual and group, ensuring feedback from the community is incorporated into planning as well as the goals of the organisation.
  • Develop partnerships with other health services and Aboriginal community groups to ensure the education and communication of health issues are culturally appropriate, easily accessible and understood by the community.
  • Support access to and participation in all HealthWISE activities and services.
  • Assist in implementing the HealthWISE plans and priorities, including health promotion activities and events.
  • Effectively collaborate with team members to ensure that all HealthWISE services and activities are operating effectively and efficiently, while maintaining a harmonious team environment
  • Record accurate notes and information to ensure compliance to all organisational policies, procedures and legislative requirements, where indicated.
  • Provide data and reports to meet organisational and statutory requirements including supporting recommendations.
  • Ensure client and community confidentiality is maintained to build and maintain trust within key stakeholder groups.
  • Complete general word processing and spread sheet requirements and develop presentation materials where necessary.
  • Assist in developing and delivering training sessions to ensure all program and clinical staff are knowledgeable and aware of relevant program information.

Role Criteria

Essential

  • Hold a Certificate III or IV in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent
  • Demonstrated attitude and behaviours that are in keeping with the company’s values of client focus, empowerment, courage and excellence.
  • Demonstrated commitment to the delivery of quality, consistent and continuous health services to local communities.
  • Demonstrated understanding of issues affecting Aboriginal and Torres Strait Islander people and their health.
  • Current unrestricted and unencumbered Driver’s Licence.
  • Willingness to travel and work after hours if required.
  • Willingness to consent to a National Police Check.
  • Hold a current Working with Children Check (NSW) or Blue Card (QLD) or willingness to obtain.
  • Maintain vaccinations as required and provide proof of vaccination if requested.
  • Demonstrated experience in being able to work independently, as well as work positively and co-operatively within a team environment as well as within tight time schedules and in accordance with variable workload demands
  • Demonstrated experience to work remotely and autonomously with accountability
  • Demonstrated experience using a computer, in particular Microsoft Word, Excel and Outlook software packages
  • Demonstrated high level written and oral communication skills including facilitating activities with a wide range of individuals and groups
  • Demonstrated organisational and high level problem-solving skills as well as the ability to support staff, consult and liaise and with GP’s and other health professionals
  • Demonstrated experience in working in interagency partnerships to strengthen the health, social and emotional wellbeing of Aboriginal and Torres Strait Islander communities.

Desirable

  • Experience working and engaging with regional Aboriginal and Torres Strait Islander communities in a primary health care setting.
  • A demonstrated knowledge and understanding of Primary Health Care as it applies to a preventative strategy and chronic disease management.
  • Practical experience in program development, implementation, management and co-ordination
  • Have experience working with health service agencies, non-government organisations, community groups and medical professionals at all levels
