About the Role

Are you an experienced case manager who is passionate about helping older people live safely, independently and with dignity in their own homes?

3Bridges is looking for a Care Partner to join our Aged Care team on a full-time, fixed term (18 months) parental leave contract based at Carss Park.

In this role, you’ll manage an allocated caseload, building trusted relationships with clients, families and carers while developing and reviewing individual care plans, coordinating services and responding to changing client needs.

You’ll work collaboratively with frontline workers, the Rostering team, and other internal services to ensure clients receive responsive, person-centred support that promotes their independence, safety and wellbeing.

Key Responsibilities

• Build trusted relationships with clients, families, carers and advocates


• Manage an allocated portfolio of aged care clients


• Develop, implement and regularly review individual care plans


• Conduct client consultations, welfare checks and home visits


• Monitor changing client needs, risks and safeguarding concerns


• Coordinate appropriate services and supports


• Support clients to achieve their individual goals


• Manage client budgets in line with funding requirements


• Maintain accurate care plans, case notes and client records


• Work collaboratively with frontline workers, Rostering and internal teams


• Ensure services are delivered in accordance with relevant aged care quality and compliance requirements

About You

You’re an experienced and compassionate case management professional who enjoys building strong relationships with clients and their families, along with a genuine commitment to person-centred care.

Essential Requirements

• Tertiary qualifications in a Human Services Discipline, i.e. Social Work/Psychology or similar area


• Demonstrated case management experience within aged care, community services or a related field


• Strong communication, organisation and relationship-building skills


• Ability to manage care plans, client records and competing priorities


• Current unrestricted driver’s licence


• Relevant employment screening and checks applicable to the role
