Position Title: Case Manager - Ashmore

Location: Ashmore Status: Permanent, Full‑Time (Site‑Based) Classification: SCHADS – Crisis Level 2 / Community Level 4

We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About the Role

As a Case Manager with The Salvation Army, you’ll provide person‑centred support across both community and residential settings, helping individuals achieve housing stability, strengthen social connections, and build the confidence and capability to sustain long‑term independence.

You’ll draw on trauma‑informed, strengths‑based and resilience‑focused practices, working collaboratively with a passionate team to deliver services that reflect our mission, values and commitment to social justice.

How You’ll Make an Impact

  • Deliver high‑quality case management aligned with TSA models and best‑practice standards
  • Conduct comprehensive assessments that identify needs, risks and vulnerabilities
  • Provide ongoing evaluation of client circumstances from intake through to exit
  • Develop collaborative, goal‑focused case plans that address barriers and support meaningful progress
  • Implement case plans with active client participation and empowerment
  • Support clients to access services, build community connections and receive advocacy where needed

What You Will Bring

  • A degree in Social Work, Welfare, Community Development or a related discipline OR extensive sector experience with strong case management capability
  • Demonstrated experience in client assessment and case management within a social services environment
  • A current QLD Working With Children Check (Blue Card)
  • Current First Aid Certificate (preferred)
  • A valid Driver’s Licence
