About the role

As a Case Manager, you will provide culturally safe, strengths based and trauma informed support to participants engaged in the Homelessness Program. You will work closely with people navigating homelessness, mental health concerns, substance use and social disconnection. You will offer practical support, emotional encouragement and pathways to stability.

You will collaborate with a multidisciplinary team, develop individual case plans and help clients access the services and community connections they need to thrive. This role is relational, hands on and purpose driven.

This permanent full-time position within the Homelessness Program, reporting to the Team Leader. Pay is in accordance with a SCHADS Level 4.

How you will make an impact

• Walk alongside clients as they work toward their goals and support them through challenges
• Complete intake and assessments that build a shared understanding of each person’s strengths, needs and aspirations
• Develop and implement case plans that empower clients to take the lead in their own journey
• Support clients to access health, community, housing and social services
• Advocate for clients with government and non-government agencies
• Maintain accurate, respectful and confidential client records
• Identify risks early and respond with care, professionalism and safety in mind
• Facilitate life skills development and group activities that build confidence and connection
• Build strong and culturally respectful relationships with Aboriginal communities and people from diverse backgrounds
• Contribute to a service environment that is safe, welcoming and grounded in dignity and respect.

What you will bring

• A qualification in Social Work, Mental Health, Welfare, Community Development or similar, or a Certificate IV with strong sector experience
• Experience supporting people facing homelessness, AOD challenges, mental health concerns or involvement with the justice system
• Confidence working with culturally diverse communities including Aboriginal and Torres Strait Islander peoples
• Strong organisational skills and the ability to manage a varied caseload
• Clear and compassionate communication and the ability to build trust quickly
• Resilience, adaptability and a commitment to trauma informed and culturally safe practice
