We provide services and supports to adults and families through accommodation, case management, assertive outreach, financial assistance, community connection and referral services.
About the role
As a Case Manager – Housing Pathways Program (Courts), this is your opportunity to support court‑referred participants to address the issues contributing to their risk of homelessness. You will play a vital role in reducing ongoing homelessness and re‑offending by providing targeted, trauma‑informed support aligned with the National Homelessness Stream Model of Care.
Reporting to the Team Leader, Housing Pathways Program (Courts) – Program One & Innovation, this is a fixed-term full-time position, contracted for 12 months, based on site in Coburg North, VIC.
Conditions
Compensation is in accordance with the SCHADS Award – Level 5.
