Job Description - Case Manager Housing Pathways Program (Courts)

We provide services and supports to adults and families through accommodation, case management, assertive outreach, financial assistance, community connection and referral services. 

About the role 

As a Case Manager – Housing Pathways Program (Courts), this is your opportunity to support courtreferred participants to address the issues contributing to their risk of homelessness. You will play a vital role in reducing ongoing homelessness and reoffending by providing targeted, traumainformed support aligned with the National Homelessness Stream Model of Care. 

Reporting to the Team Leader, Housing Pathways Program (Courts) – Program One & Innovation, this is a fixed-term full-time position, contracted for 12 months, based on site in Coburg North, VIC. 

Conditions 

Compensation is in accordance with the SCHADS Award – Level 5.

How you will make an impact 

  • Assess immediate and longterm housing needs for clients exiting court or bail environments. 
  • Connect clients with appropriate housing options including crisis accommodation, transitional housing, rooming houses and social housing. 
  • Advocate for clients to access support services, community resources and financial assistance allocated to the program. 
  • Develop and monitor individual case plans using a traumainformed, clientled approach. 
  • Build and maintain strong relationships with Court Services Victoria, THM providers, real estate agents and community agencies to ensure flexible, accessible and culturally sensitive service delivery. 

What you will bring 

  • A tertiary qualification in Social Work or a related community services field (degree preferred; significant experience considered). 
  • Demonstrated experience supporting individuals experiencing or at risk of homelessness. 
  • Strong understanding of homelessness models, legislation, sector context, and bestpractice approaches. 
  • Knowledge of the justice system and experience working within correctional or courtrelated environments (highly desirable). 
  • Experience working collaboratively with a range of stakeholders, networks and community agencies. 
  • A current Driver’s Licence  
  • A current Victorian Working with Children Check (Employee).
