ABOUT THE ROLE:

We are excited to welcome you to the St Vincent’s Private Hospital Melbourne (SVPHM) team as a Social Worker within our Continuum of Care Department.

This position is based across our Fitzroy, East Melbourne, Kew, and Werribee campuses.

This is a Grade 3 Social Worker role comprising 3 days per week and designed to provide support to our post discharge care team.

ABOUT YOU:

As an outstanding Social Worker, you will bring:

• A strong understanding of the diverse issues affecting Aboriginal and Torres Strait Islander peoples
• The ability to work autonomously and also within a multidisciplinary team to support the discharge of patients with complex social needs
• A minimum qualification of a Bachelor of Social Work
• Five + years of experience working as a Social Worker in a hospital environment
• In-depth knowledge of the Aged Care and Community Services  sector
