Job Description - Manager Care Planning and Practice (MCPP)

Manager Care Planning and Practice  


Location: Tingalpa, Brisbane, QLD  


Description of the Job Role 


The position of Manager Care Planning and Practice is responsible for managing a caseload of roughly 12-18 Child Safety and Disability Support clients. In this role, the Manager Care Planning and Practice’s primary responsibility is to provide professional care management advice and support to the principals and other members of the group, to ensure that the design, implementation, management and evaluation of professional care practices have met relevant standards 


Employment Conditions 


The normal working hours will be 9:00am to 5:00pm Monday to Friday, although there may be some variation from time to time to ensure critical management and continued care of clients, not limited to and includes the on-call roster.  


 


Reporting Relationship 


This role will report to Senior Manager Care Planning and Practice 


 


Responsibilities 



  • Provide high-quality, professional care management advice and assistance to the DCR Principals and the Executive Manager (Care Policy and Practice) particularly about adults with an intellectual disability some of whom may have a dual diagnosis or be subject to forensic orders, children, and young people subject to Child Protection Orders arrangements and people who have a chronic illness.  



  • Provide professional advice and support to the Executive Manager (Care Policy and Practice) and the Principals, particularly when undertaking the development, write-up, implementation and ongoing management of Incident Reports, Individual Personal Plans (IPPs) for assigned clients with a disability and children and young people under Child Protection Orders.  



  • Read and provide feedback to Behaviour Support Practitioners on the accuracy of information, practicality, and implementation of Personal Behaviour Supports Plans (PBSPs).  



  • Represent DCR at hearings of the Queensland Civil and Administrative Tribunal (QCAT) in matters associated with, for example, guardianship, financial administration and restrictive practices and other relevant client issues falling within the jurisdiction of the Tribunal.  



  • Develop in association with other health professionals, care treatment regimens for clients with medical, mental health and/or complex developmental and behavioural issues  



  • Conduct regular supervision and training of DCR care staff on-site and prepare appropriate reports and documentation.  



  • Participate, on regular rotation, in an ‘emergency after-hours’ contact roster to provide professional advice to DCR care staff in managing critical incidents affecting clients and client care. 



  • Develop in association “collaboration” with other health professionals, care treatment regimens for clients with medical, mental health and/or complex developmental and behavioral issues. 



  • Ensure compliance with Human Resource Quality Framework and NDIS Practice Standards and Quality Indicators 



  • Ensure the appropriate use of client and DCR finances 



  • Engagement with a variety if stakeholders to ensure service delivery is tailored to the individual needs of the client 


Individual Productivity Targets  



  • Deliver household supervision every 4 weeks.  



  • Maintain all direct reports compliance  

  • Ensure all incidents are reported in the correct timeframe 


Person Specifics 


Desired person will have Tertiary qualifications in Psychology, Nursing, Human Services, Social Work, Special Education, or similar fields.  


Relevant experience or Postgraduate qualifications would be highly advantageous. 


Demonstrated comprehensive knowledge of legislation, policy, and practice in a least one (1) of the following areas:  



  • Disability Care  



  • Forensic Disability  



  • Child Protection  



  • Aged Care  



  • Health Care 


Possession of a:  



  • Driver’s license  



  • Proof of Vaccination for COVID-19 by DCR policy  


Possession of or willingness to obtain:  



  • First Aid & CPR  



  • Blue Card  



  • NDIS Worker Screening Check  



  • LCS2 Background and History Check
