Job Description - MND Advisor and Support Coordinator Team Leader

MND Victoria provides expert information, advice and support services for people living with motor neurone disease (MND) in Victoria.

We have a fabulous team of 21 MND Advisors & Support Coordinators supported by 3 Team Leaders who work with people diagnosed with MND across the Victoria and Tasmania. One of our team leaders has taken up another opportunity in the organisation, hence the reason for this vacancy!

We are seeking an experienced health professional, ideally with an OT, physio, speech therapy, social work or nursing background to provide leadership, guidance and supervision to lead a small team of 7 to 8 MND Advisors/Support Coordinators, all who are qualified health/disability professionals themselves. This position also has a direct service delivery component (approximately 40% of the role) and will provide advice and support coordination to people with MND.

Half of the people with MND that MND Victoria support are under the age of 65, receiving most supports through the NDIS and half are over the age of 65, hence receive supports via My Aged Care. This adds to the variety and diversity of the work.

We require the successful applicant to have experience working with people living with a progressive degenerative neurological disease, however, are able to offer extensive training about Motor Neurone Disease itself.

THE SUCCESSFUL APPLICANT WILL HAVE:

  • A tertiary heath professional qualification
  • Experience supervising, supporting and developing staff
  • Experience in working with people living with a progressive degenerative neurological disease
  • A “can do approach” with a demonstrated client focused approach to all work activities
  • An interest in providing team support as well as maintaining a small caseload

This is a full-time position, but we will consider applicants who can work .9FTE. Excellent salary (range $117,000 to 123,000, pro rata + superannuation), own work motor vehicle and salary packaging available.

Essential

  1. A relevant tertiary qualification in a health/welfare related discipline
  2. Demonstrated experience supervising, supporting and developing staff
  3. Demonstrated experience in working with people living with a progressive degenerative neurological disease
  4. Demonstrated experience and knowledge of support and care planning, assessment and referral in the disability and aged care sectors 
  5. Demonstrated understanding of the impact of progressive degenerative neurological conditions on family and carers
  6. Highly developed interpersonal communication skills, both verbal and written
  7. Demonstrated experience working independently and as part of a team
  8. Demonstrated negotiation and creative problem-solving skills
  9. Demonstrated ability to manage competing work tasks and work effectively to deadlines
  10. Demonstrated computer literacy and understanding of electronic databases
  11. Confidence and experience in using virtual technology to engage with clients, team ambers and service providers
  12. Demonstrated ability to develop relationships with service providers and identify appropriate services to meet client needs
  13. A “can do approach” with a demonstrated client focus to all work activities
  14. Demonstrated interest and experience in delivering presentations to groups of people.
  15. Current Victorian Driver’s Licence

Desirable

  1. Project management qualifications &/or experience. 
  2. Ability to demonstrate experience in identifying and implementing service improvements 
  3. Demonstrated experience providing support coordination to NDIS participants
  4. Demonstrated knowledge and experience in working in the aged care sector.
