Who are we?
Aged Care Assessment - Grampians (part of Grampians Health) is a well-regarded aged care assessment organisation, that provides community and hospital based aged care assessment across the Grampians Region.
About the Role
The purpose of this role is to undertake personal Home Support assessments for My Aged Care clients. The Home Support assessment is designed for people who may need entry-level support to help them stay at home. Non-Clinical Assessors incorporate the Aged Care Assessment Quality framework to deliver high quality assessments, with a focus on client experience. The aim is to work with clients to develop support plans to achieve wellness and healthy ageing goals through linking and referral to the client's chosen support services through the My Aged Care Gateway.
