Pharmacist in General Practice

Contribute to the development and implementation of innovative models of care to improve patient outcomes

Assist in the development and review of care plans for patients with chronic conditions

Conduct health assessments and coordinate care for patients with complex needs

Conduct medication reviews and provide advice to patients and healthcare professionals on medication management

Develop and implement medication management plans in collaboration with the general practitioner and other healthcare professionals

Work collaboratively with general practitioners, practice nurses, and other allied health professionals to optimise patient care

Facilitate case conferencing with general practitioner, non-GP specialists, practice nurse, and other allied health professionals

Provide education and training to patients and healthcare professionals on medication management and safe use of medications

Monitor and manage medication-related problems and adverse drug reactions

Ensure compliance with medication-related regulations and guidelines

Bachelor of Pharmacy degree or higher

Current registration with the Pharmacy Board of Australia

Minimum of 2 years of experience as a pharmacist

Excellent communication and interpersonal skills

Ability to work collaboratively with a multidisciplinary team

Ability to prioritise tasks and manage time effectively

Ability to work independently and pivot to varying tasks
