About the Role:

As the Practice Lead, you will be focusing on ensuring each person benefits from person-directed plans and receives consistent, competent supports delivered by skilled and passionate staff. 

You are a specialist in support delivery and an expert in translating theory into practice, that goes beyond simply putting systems in place and filling in paperwork.

You will lead, develop, encourage, unite and inspire a staff team to put into practice the vision that values the individuality and rights of all people to live a fabulous life ‘The Melba Way’.  

Responsibilities: 

  • Uphold and protect the human rights of persons supported, ensuring their safety and well-being. 
  • Coach staff in good practice through feedback and role modelling good practice, enabling individuals to engage in daily routines and desired outcomes.
  • Demonstrate effective practice leadership by guiding and supporting team members, including during delivery of support. Focusing leadership on staff support for the person supported ‘quality of life’.
  • Lead and supervise staff to implement support plans and meet key objectives, including daily oversight and performance management.
  • Ensure staff assist persons supported with: Developing and maintaining important relationships; and pursuing social, educational, vocational, and recreational opportunities. 
  • Foster partnerships with various service providers to create opportunities for those accessing support services. 
  • Stay updated on relevant legislation, NDIS Service Standards, and best practices through ongoing professional developments. 
  • Assist in recruitment processes as needed. 
  • Partner with the Relationship Partner to maintain the health plan and positive behavioural support needs for each supported individual. 
  • Partner with the Relationship Partner to document and maintain relevant information regarding supports and outcomes, including Personal Outcome Measures (POMs). 
  • Work with the Relationship Partner to ensure that incident reports and complaints are responded to quickly and sensitively, as well as ensuring that learnings are shared promptly with support staff and other services.   

Your Background (Skills and Attributes):

  • A track record of effectively leading teams of staff to perform their roles to a high standard and to provide innovative and progressive service delivery models.  
  • An understanding of both physical and intellectual disabilities and other complex presentations (including mental health support).  
  • An excellent knowledge of and experience with the Person Centred Active Support framework.  
  • A good understanding of Positive Behaviour Support (PBS) and person-centred practice. 
  • A strong understanding of when and how to engage allied health or other professional expertise to ensure the health and well-being of the person supported. 
  • Strong interpersonal skills and ability to form effective working relationships with colleagues, management, families, and external bodies. 
  • Capacity to follow instructions, policies, and procedures. 
  • Willingness to collaborate and build positive relationships to achieve common goals. 
  • Bring a flexible, positive, and self-motivated mindset to your interactions every day. 
  • Certificate IV in Disability (desirable, not mandatory). 

Requirements:

  • A current NDIS Worker Screening and Working with Children Check (we can support you through the process of obtaining one or both checks if you don’t already have them). 
  • A current First Aid and CPR Certificate. 
  • The ability and relevant driver’s license to perform travel and work a variety of shifts, including weekends. Position may also require working active nights. 
  • Compliance with OHS regulations, legislation, and Policies and Procedures is required. Successful completion of pre-employment safety screening checks, mandatory training, and their subsequent renewals is necessary.
