About the job:

  • This is an opportunity for a Registered Nurse with extensive knowledge and experience in Palliative Care to join our dedicated team.
  • In this role you will work Monday to Friday am – pm commencing from the Toronto CHC.
  • Clinical Handover is to be delivered at the commencement of duty following days off.
  • Case planning and care management is to be organised by Clinicians and diary appointments maintained using the CHIME program.
  • Clients will be seen face to face, via telehealth or via phone contact.
  • In the afternoons you will complete documentation, case management and coordination with other services involved in client care.
  • On average three (3) - Four (4) clients are seen per Clinician per day. Two (2) Clinicians are on duty Monday to Friday. One (1) Clinician is on duty at weekends.
  • Telephone on-call to current clients is provided seven (7) days per week and shared between Clinicians.
  • Community Palliative Care Nurses can engage in reflective practice and supervision in collaboration with the John Hunter Hospital Department of Palliative Care or individual supervision as preferred.

  • Requirements: Our successful candidate will possess the following:

  • Current registration as a Registered Nurse with AHPRA.
  • Provide high quality, holistic and client-centred care in the community setting. This care extends to clients and their carers and includes clinical assessment, symptom management, provision of psycho-social support, care planning, evaluation, and end-of-life care, throughout the course of the illness. 
  • The applicant would need to provide extensive expertise in this area when complex symptoms/needs exceed what can be managed by the existing care provider/s.
  • The Specialist Palliative Care Nursing Service must complete - as required :
  • HNE LHD Corporate Orientation in line with NSW Health Policy.
  • CACS GNS Induction Program.
  • Other education pathways as advised.
  • An induction program will include core clinical skills, processes, pathways, and orientation to the clinical areas.
  • Learning pathways and the required mandatory training can be found within:
  • Palliative and End of Life Care Development Pathway.
  • End of Life navigator-My Health Learning.
  • Other Ad Hoc training is available via the Palliative Care District Education programme, CACS GNS Staff Broadcasts and as advised in My Health Learning by the Nurse Unit Manager.

  • An eligibility list will be created for future permanent full or part time and temporary full or part time vacancies.
  • This position is full time; however, part time/job share arrangements may also be considered.

  • Information for Applicants:

    All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health or provide an approved medical contraindication certificate (AIR Immunisation History Statement) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

    Acceptable proof of COVID-19 vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement (IHS), or AIR COVID-19 Digital Certificate, or AIR COVID-19 and Influenza IHS. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
