About the opportunity
The Service Coordinator is an integral role in the functioning of the Mercy Health Home Care Service through the provision of service coordination for Home Care clients. The position is responsible for coordination and scheduling of care and services to clients living in a home care setting. The client group is varied and includes but not limited to:
• Support at Home
• CHSP clients
• Brokered services
• DVA/VHC clients
• Private- fee for service clients
• NDIS clients
Reporting to the Team Leader, the Service Coordinator position will work closely with the Care Advisory team within the designated geographic region and have responsibility for ensuring that clients receive the right service in a timely manner in line with the Aged Care Act 1997, Home Care Standards, consumer directed care principles and Mercy Health policy and procedures.

Experience in rostering systems and coordination
Advanced experience in client management systems and extensive knowledge of Microsoft Office
Excellent customer service skills including presentation and communication
NDIS Worker Screening Check Clearance
