We are currently seeking a SINC Administrator to support the delivery of Aged Care services funded by the Commonwealth Home Support Program (CHSP). 

Working closely with the Home Care Packages Team, you will be responsible for the maintenance, growth, and development of SINC Program while supporting older Australians to remain living safety in their homes and connected to local communities. 

 

The Role

As the Seniors in Networked Communities (SINC) Administrator, you will work across the Canberra & Goulburn Archdiocese, in the ACT and Southern Highlands Regions to ensure the delivery of aged care services under the Commonwealth Home Support Programme (CHSP). You will be required to work within a person centred, wellness and reablement or restorative framework as outlined in the CHSP Manual. All services are provided in line with the Aged Care Quality Standards.

 

Responsibilities include but are not limited to:

  • managing referrals and the client waitlist;
  • on boarding clients to the program;
  • development of care plans, risk assessments and other relevant documentation 
  • liaising with the rostering team;
  • liaising with clients and families;
  • ensuring that services are consistent with My Aged Care support plans; and
  • referring to My Aged Care should people’s needs change.

Ideal person

The ideal person we are looking for has:

  • Certificate in Aged Care or similar and relevant experience.
  • High computer literacy, e.g. Microsoft Office and use of a client management system.
  • Good communication and interpersonal skills. 
  • High level of attention to details and a quick learner. 
  • Ability to juggle multiple tasks with effective time management skill and meet deadlines.
  • Be in possession of a Working With Vulnerable People’s (WWVP) card (for ACT Staff) and be willing to undergo a National Criminal History Check.
  • Current driver’s license and vehicle with comprehensive insurance.
