About the role
Monday to Friday role with on-call requirements, predominantly working in the Heart Lung Stream, providing social work services to patients, families, and carers within Cardiology , Cardio-Thoracic Surgery, Thoracic Medicine, Thoracic and Cardiac Rehabilitation and Heart and Lung Transplant including psychosocial assessment, risk assessment, counselling, and family support
Participate in multidisciplinary team meetings to support patient care, discharge planning, and coordination of referrals to community supports, accommodation, and placement services
Advocate for patients and carers, including support with NCAT Guardianship applications and NDIS/NDIA processes where required
Contribute to clinical operations including documentation, handovers, statistical reporting, clinical cover, on-call duties, and resource development
Engage in professional development activities including supervision, education, research, student training, quality improvement, and mandatory training compliance
About you
Hold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW), with demonstrated clinical experience in a tertiary hospital or health-related setting
Strong clinical skills and knowledge in psychosocial assessment, counselling, therapeutic interventions, case management, advocacy, and complex discharge planning
Understanding of and practical experience in crisis intervention, short-term solution-focused approaches, and grief and loss support
Ability to work effectively within multidisciplinary teams in high-pressure environments, managing competing demands with strong negotiation and liaison capabilities
Willingness and ability to participate in extended hours and on-call service as required
