About the role 




Join the Swanston Centre as a Social Worker and make a meaningful difference to the recovery and wellbeing of consumers experiencing major mental illness across the Barwon region.


Working within a 32-bed inpatient unit, you will be part of a multidisciplinary therapeutic team delivering individual, family and group interventions. You will have a particular focus on supporting consumers with complex needs, working collaboratively with community case managers, primary nurses and external services.


You will contribute your social work expertise to assessment and treatment planning, with a focus on addressing the broader factors that impact recovery, including housing, employment, financial wellbeing, family relationships, dependent children, peer connections and social isolation.


This permanent part-time role offers an opportunity to establish and further develop the Social Work function within the inpatient setting, while contributing to a recovery-oriented approach that supports consumers to achieve their goals and reconnect with their communities.


 



 


Key Responsibilities 




  • Deliver high-quality, evidence-based social work interventions, including psychosocial assessment, counselling, advocacy, education and support for consumers with complex needs throughout admission and discharge planning

  • Work collaboratively within a multidisciplinary team to provide individual and group interventions, coordinate care with community teams and services, contribute to clinical reviews, and provide social work education and support to colleagues

  • Contribute to professional practice, quality improvement and service development, including clinical supervision, student placement, accurate documentation and data collection, risk management, and continuous improvement initiatives



 




What we’re looking for 




  • AASW-accredited Bachelor or entry-level Master of Social Work, eligibility for AASW membership and a current Victorian Driver’s Licence

  • Demonstrated clinical social work skills and experience, including psychosocial assessment and intervention with people experiencing major mental illness, with a strong understanding of recovery-oriented and trauma-informed care

  • Sound knowledge of mental health practice and relevant legislation, including the Mental Health and Wellbeing Act 2022, MARAM, psychiatric treatment approaches, inpatient care and discharge planning, including NDIS pathways
