Job Description - Support Coordinator

The Opportunity

This full-time position is based in our new head office in Mooloolaba.

Thrive House focus on the needs of participants and let their needs and abilities guide our practice. Our person-centered approach seeks to empower the participant to direct the intervention and assess what resources they need.

We’re Looking For: 

  • Someone who is resilient, caring and passionate about providing quality care to a diverse range of people.
  • Relevant tertiary qualifications or studying towards in behavioural or human services (for example, psychology, social work or similar).
  • 1 - 2 years experience in a Support Coordination role, ideally within the NDIS, Child Protection, and/or Mental Health sectors (desirable).
  • An understanding of NDIS plans, support budgets, NDIS support catalogue and NDIS Pricing Arrangement.
  • Ability to develop and maintain good working relationships with agency staff, participants and service providers.
  • High level understanding and knowledge of disability and alternative care frameworks, including knowledge of the Disability Services Act 2006 / NDIS Act 2013.
  • Demonstrate ability to collect data, plan, prepare and complete written reports in a professional manner for plan review purposes and unscheduled reviews.
  • Understanding of HSQF guidelines.
  • A commitment to strength based, solution-focused and crisis intervention techniques.
  • Experience working in crisis situations with a trauma informed approach and understanding of various evidence-based practices.
  • You will be committed to training and continuous development with a desire to Thrive!
  • You are eligible to work in Australia and you hold a current QLD driver’s licence and are eligible to obtain an LCS check, blue card and yellow card.
