The Way Back is a free, confidential support service that bridges the gap between hospital discharge and long-term wellbeing. Our support is grounded in connection by building trust, restoring hope, and helping people rediscover their own strengths and purpose.
This role is fixed term until July 2026.

Your Role as Support Facilitator
You’ll deliver holistic non-clinical follow-up care, practical support and outreach to people directly after a suicide attempt by:
Ensure individual and team practice complies with relevant legislation and national mental health and recovery frameworks
Provide leadership and direction on psychosocial rehabilitation, social inclusion, and recovery-oriented practice
Offer operational support and leadership in the Program Manager’s absence
Provide mentoring and consultation to team members as required
Triage referrals and provide professional consultation
Conduct psychosocial assessments and collaboratively develop, implement, and review flexible, individualised recovery plans
Provide assertive outreach and practical, in-home and community-based support
Facilitate access to and linkage with ongoing community and primary care supports
Identify and connect clients with resources supporting social, educational, vocational, and community integration
What We’re Looking For
Tertiary qualifications in community, social welfare, health or a related discipline
Experience working in senior roles within a recovery orientated service framework for mental health and/or suicide prevention
Passionate about making a positive change in the lives of people managing a suicidality
Deep understanding of the principles of recovery in the mental health setting.
Demonstrated experience working with people with complex issues associated with those experiencing suicidality
Capacity to work independently and collaboratively within a team environment including being accountable for the effective delivery of client services
Sound communication and interpersonal skills including the ability to engage, negotiate, advocate, collaborate and undertake joint innovative problem solving
A Valuable Addition, Yet Not a Must-Have!
Experience providing care coordination within a mental health setting
Understanding of the principles and standards of the Disability Services Act (Qld 2006) and the National Mental Health Standards and how they relate to providing effective and accountable services
Essential Requirements
Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
Relevant probity checks required by legislation and WMQ Limited policy.
Current Driver’s Licence and willingness to travel for work
