Job Description - Supported Living Facilitator

Supported Living Facilitator | Disability Services, NDIS  

  • Employment Term: Casual

  • Location: Gold Coast

  • Remuneration: $43.23 p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

    Your part in our UnitingCare team   

    • Work in partnership with the person being supported, their family and informal support networks, to attain the hopes and aspirations they have established for their lives 

    • Provide support to people in their everyday living situations, including personal care as identified, with an emphasis on choice, enhancing quality of life 

    • Participate in the development, implementation and review of individual plans that focus on the life hopes and aspirations of people 

    • Enable people to attain personal autonomy over their own lives and the services which they receive 

    • Recognise and uphold the right to respect, privacy, dignity and confidentiality of the person being supported and their family 

     

    What makes a difference for us?    

    • A relevant Industry qualification (e.g. certificate 3 in Disability or individual Support) is desirable. 
    • Available to work Weekend and Sleepover shifts
    • Experience working in the Disability, Aged and Community sector is ideal but not essential – as long as you have positive attitude and a willingness to learn  

    • A current NDIS Worker Screening check and Blue Card (or in the process of obtaining) 

    • Current Open Drivers License 

    • Current CPR and First Aid Certificate ( before you start)  

    • Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers  

    • Being flexible (able to work varied hours and days) and reliable to meet the needs of the people we support  

    • Able to effectively communicate in spoken and written English
