Job Description - Team Leader - Intensive Support Program



  • Location: Maribyrnong Community Mental Health and Wellbeing

  • Employment Type: Ongoing, Full Time

  • Salary: RPN4/OT4/SW4/P3 + Superannuation + Salary Packaging


 


About the role:


We are seeking an experienced and motivated clinician to lead the Case Management, Intensive Support Program (ISP) within the Maribyrnong Community Mental Health and Wellbeing Team.


 


ISP provides time-limited, high-intensity community support for people experiencing complex mental health needs who require more intensive engagement than standard case management. The program focuses on stabilisation, intensive intervention and supporting consumers to transition back to longer-term supports.


 


This is an exciting opportunity for a Senior Clinician who is passionate about recovery-oriented care and supporting staff to deliver high-quality mental health services in the community.


 


The Melton Community Mental Health and Wellbeing Team provides services to a rapidly growing and diverse community in Melbourne’s western region. You will be joining a supportive service that values collaboration, innovation and delivering high-quality care to the local community.


 


Key responsibilities:



  • The Team Leader provides clinical and operational leadership to a multidisciplinary team delivering intensive, community-based mental health care.

  • Providing clinical leadership, supervision and support to team members

  • Managing referrals, service flow and caseload allocation

  • Monitoring clinical risk and supporting safe practice

  • Promoting recovery-oriented and trauma-informed care

  • Working closely with intake, ACIS and community teams to support smooth transitions for consumers

  • Supporting staff development, performance and a positive team culture


 


About you:



  • You are an experienced mental health clinician with strong leadership skills and a commitment to high-quality community mental health care.

  • Registration as a Registered Psychiatric Nurse, Psychologist, Occupational Therapist or Social Worker

  • Demonstrated experience working with consumers with complex mental health needs

  • Strong clinical leadership and decision-making skills

  • Experience providing supervision or leadership within a multidisciplinary team

  • Commitment to recovery-oriented and person-centred practice
