Job Description - Transition Coach

  • Two positions available 
  • PERM full time – 38 hours per week

$90,188.13 + super + nfp bens

  • MAX Term until 30/06/2027 (parental leave cover) part time 22.8 hours per week

$54,113.04 + super + nfp bens

  • Location:  Goomburrup (Bunbury) WA 6230

Your Opportunity 

The Home Stretch WA program, a groundbreaking initiative co-designed with young people, Aboriginal community members, and the broader support network, is dedicated to transforming the transition from Out-of-Home Care to independence.

As a Home Stretch Transition Coach Case Manager based on site in Bunbury with Mission Australia, you will play an integral role in this innovative service.

In this rewarding role, you will provide individualised, professional support and case coordination to young people, extending crucial resources and guidance as they navigate the complex journey to independence. By offering continued assistance up to the age of 21, you will help ensure that these young people have the stability, skills, and confidence they need to thrive in adulthood. Your work will contribute to better outcomes for these individuals and help shape a more effective and supportive system for future generations.

We have two positions available – please indicate your preference in your application.

Your Key Responsibilities:

  • Develop a young persons’ natural networks and assisting them to build and maintain connection to culture, country, family, and community supports.
  • Offer and broker access to a safety net of financial support and resources through the Home Stretch WA service offer.
  • Provide tailored support services using a culturally safe and strengths-based approach and case coordination that is consistent with the Home Stretch WA Transition Coach Practice Principles.
  • Working collaboratively with relevant stakeholders including the Department of Communities Child Protection staff, foster families, carers, the young person’s kin, community, and people identified as significant in their lives.
  • Ensure the right to self-determination drives your practice, while promoting the wellbeing of young people.

Requirements for Success 

  • A Degree or Diploma in Social Welfare/Social Science or similar and a minimum of two years’ experience in a similar role
  • Comprehensive understanding of the barriers and issues affecting young people who have been in state care, including those disengaged from education, training, and employment.
  • Ability to work in a culturally safe, trauma-informed manner, with strong knowledge of Aboriginal culture.
  • Proven experience in providing case management, assessment, counselling, and crisis intervention for vulnerable young people with complex needs.
  • Strong interpersonal and networking skills, with the ability to build effective relationships with Aboriginal communities and services.
  • Excellent written and verbal communication skills, along with high-level computer literacy.
  • Ability to work flexibly in a solutions-focused manner, both independently and as part of a diverse team.
  • Additional WA CPFS Child Protection Screening Checks
  • Must have a Full driver licence with no restrictions.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
