About Us

“My Care Circle” is a "Service for One". We are not a traditional service provider but a bespoke NDIS participant family owned 24/7 service to a single client. The client is a 23 year old young man who requires a 2:1 continuous care service. There is a Registered Nurse/ Paramedic and Support Worker on each shift. The Team  consists of approximately 16 staff and provides care in a self provided Specialised Disability Accommodation home for one client only. This is not a group home situation but a bespoke care service for one special young man. If you join “My Care Circle” you will be assured of regular work with flexibility built into  the role. No running between clients for a few hours here and there. We believe the client will only get the stable, sustainable service he  requires if staff are treated well and paid competitively. There are 340 hours a week of support to be provided to the client. Should you join  "My Care Circle" you will be  responsible for the empowerment, administration, training and management of  an existing team that includes Registered Nurses/Paramedics and Disability Support Workers. This is a person centred care model that is a combination of a  clinical model of care and a disability model of care. The goal of the service is to provide high quality care that meets the medical needs of the client as well  in a way that empowers him to live his best life. He is a very active, fit, social, outgoing young man and  this role is a leadership role that  leads his Team to support him to achieve his goals in life in a way that is positive and  empowering,

About the Role

You will be paid above award wages to lead a  Team  of professionals to provide in home support and community access supports so that the client can live his best life. You will be trained by family to meet the specific needs of the client and will be paid to attend formal training sessions to give you the skills and expertise to lead the Team. Your role will be to  ensure the clients needs are met by the Team in a continuous  model  of 24/7 care. You will  work closely with the Team to ensure that staff are supported onsite in their role with the client and that  staff competency to deliver the service is assessed and any gaps  in training identified and training delivered in a  safe, sustainable and stable service to the client.  The position involves  an average of 60 hours a fortnight work. Of those hours 20 hours are notional hours delivered around house needs so are flexible.  An on call allowance is paid one week in two.  This is a clinical role, as well as a management role to ensure care is delivered to the client and the home environment and garden is maintained by the Team in conjunction with the client. This is a rare opportunity to focus on one client and make a difference in his life. It is a wholistic care situation so requires flexibility and ingenuity. This is not an institutionalised setting , nor is it a hospital ward… it is a young mans home and his safe place.

Position Type

Part time permanent is expected to be offered  to the suitable applicant upon completion of the probation period.

About You

You will  be an experienced Registered Nurse or Paramedic with proven experience in leading and managing a Team.  You will hold a Bachelor of Nursing or Bachelor of Paramedicine and be APHRA registered. The position is responsible for delivery of the overall management and steering of the Team to achieve  the outcome of a stable, safe and sustainable service to the client. The position would suit  nurses with Nurse Unit Manager experience or Clinical Management experience in nursing or paramedicine in a clinical setting where you have had to use highly developed staff management skills to empower and manage staff to deliver a quality care clinical service.  Experience in the delivery of training to clinical staff would be an advantage but not mandatory. ED, ICU and or mental health nursing management experience  or Paramedical Management experience would be an advantage. You will be a Team  player  and a Leader who leads by example. You will show commitment to delivery of the service in a way that displays honesty, integrity, initiative and proactivity. You must be a self-sufficient leader of people. You will have excellent written and communication skills. You will have high level computing skills. You will have excellent time management skills and be a problem solver and an initiator.

Experience in airway management and/or seizure management and or behavior management is highly valued but not mandatory as you will be trained to the clients specific needs by family and via training programs. Lived disability experience is also valued. An understanding of how clinical practice intersects with disability care, empowerment and choice and control is very important as this model of care includes both the clinical model of care but also the disability model of care. Overall you need to be a positive, enthusiastic person who drives the Team forwards as a group to empower the client to live his best life despite his medical and disability complexities.
