Job Description - Clinical Nurse Consultant (Diabetes)

Your role at St John of God Subiaco Hospital

St John of God Subiaco Hospital is seeking a highly skilled and dedicated Clinical Nurse Consultant (Diabetes) to join the Allied Health team on a Casual basis.

The CNC Diabetes is accountable for the delivery of exceptional nursing health care and education for the Diabetes Service. With the SJGHC Foundations of Excellence at the centre, the CNC Diabetes provides patient-centred nursing care that is

The Position:

  • Maintain advanced clinical skills and knowledge in diabetes management.

  • Assess and facilitate the coordination of care for patients with complex diabetes management issues.

  • Apply advanced problem solving to diabetes management aspects of patient care and assist nursing caregivers to improve their problem solving and decision-making skills.

  • Promote and facilitate a multidisciplinary approach to the delivery of patient care.

  • Provide clinical leadership and clinical consultancy to the multi-disciplinary team, providing guidance and direction to resolve diabetes management issues.

  • Develop, implement and evaluate relevant clinical standards and policies using an evidence-based approach.

  • Initiate and implement research based nursing practice in areas of clinical specialty.

  • Provide education/advice to patients and significant others. Develop education resources and arrange specialised education as required.

  • In conjunction with LOD develop, implement and evaluate in-service education for caregivers.

You will be a Registered Nurse with current registration with the Nursing & Midwifery Board of Australia (AHPRA), supported by a post‑registration qualification in Diabetes Education as recognised by the Australian Diabetes Educators Association, or be actively working towards this.

You will bring extensive post‑registration experience along with expert knowledge and advanced clinical skills in diabetes management. With a proven ability to lead a strong clinical safety and quality culture, you will place patients at the centre of care, optimise clinical outcomes, and contribute to an exceptional patient experience.

  • Casual Hourly rate: $86.57 (incl. casual loading), plus 12% superannuation
