3FW is a 24-bed general medical inpatient unit (IPU), incorporating a dedicated 8 bed Close Observation Area (COA), for vulnerable patients experiencing changes in their cognition and behaviour, related to delirium and or dementia.
A primary focus of the service on 3FW is to provide high quality care to general medical patients who have complex care needs relating to age and frailty. The COA Pod uses a nurse led team approach to provide increased support and care planning to maintain and improve function through tailored engagement activities.
3FW provides patient centred, evidence-based care to patients with complex or multi system and single system disease. Nursing staff work closely with the medical and allied health teams to deliver a safe, effective and efficient service in a professional and caring manner to support the best outcomes for the patient and their family.
Clinical Nurses (CN) on 3FW will be supported to develop valuable skills and knowledge related to this vulnerable and complex cohort as well as a strong leadership focus to support the NUM and Team.
The CN is expected to be able to practice autonomously and provide leadership in clinical decision making. They have the responsibility of professional leadership in the absence of the Nurse Grade 7 and an opportunity to work collaboratively to lead practice changes and clinical innovation.
