About the Role
An exciting opportunity currently exists for a Grade 2 dietitian to join the Health Independence Programs team. The Grade 2 dietitian will deliver high quality person-centred, goal-orientated care within a supportive multidisciplinary team.
This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.
Classifications for this position will be AJ1 – AJ4 ($94,478.80 - $106,906.80 per annum, pro rata for part time).

- Minimum of four (4) years post-graduate experience in a relevant field of clinical dietetics
- Demonstrated highly developed clinical management skills
- Understand and support the principles of HIP continuum of care for clients, particularly in relation to individualized, time-limited, goal-centred episodes of care that aim to improve health outcomes
- Highly developed skills of organisation, planning and problem solving
- Highly developed interpersonal skills that incorporate the qualities of excellent communication and leadership
- Current Victorian Driver’s Licence

About Department
This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.
SVHM HIP consists of integrated hub sites across several locations including Fitzroy, Kew, North Richmond and Fairfield. HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH). Community Rehabilitation is provided from our centres at Kew and Fairfield.
The key aim of Community Rehabilitation is to help people who are recovering from traumatic injury or illness, are frail or chronically ill to regain optimal function and to allow people to maximise their independence.
The position also reviews patients from the specialist clinics – Continence clinic, Falls clinic and Osteoarthritis clinic.
The programs offer a supportive and dynamic team environment which focusses on patient-centred care.

Position Description
3. LOCAL WORK ENVIRONMENT
This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated
and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning. SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield. This position is based in the HIP Community Rehabilitation Services. Community Rehabilitation provide time- limited, goal–orientated care for people who have a physical disability, are frail, chronically ill or recovering from traumatic injury or illness to regain and /or maintain optimal function. Community Rehabilitation services comprise of Community Rehabilitation Centres at Fairfield and Kew and Rehabilitation in the Home. This position also sees clients for the Continence service and Falls and Balance clinic.
This position may require working from any of the SVHM HIP sites and may be required to work within any of the HIP programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.
The position maintains positive links with the Nutrition Department and contributes to some portfolio and clinical support activities to contribute to quality activities and services to all areas.
4. POSITION PURPOSE
To work within a flexible, interdisciplinary structure, that ensures the delivery of high quality nutrition services to all clients of HIP Rehabilitation services, making sure that the client’s needs are met and they receive ‘the right treatment in the right place at the right time’. Clinicians are required to provide assessment and management of clients in a number of settings across
the continuum of care within HIP, including client’s residence, community settings, the HIP Centres or Specialist Clinics.
5. POSITION DUTIES
Assess and manage all assigned clients as appropriate, according to HIP, Nutrition Department
and St. Vincent’s policies and procedures
Provide appropriate, timely and effective clinical management to all patients through
assessment, treatment / management, re-evaluation, documentation, team liaison and
discharge planning
Provide group education related to nutrition for relevant groups of community rehabilitation
Communicate client issues effectively with other team members, with active contribution to
multidisciplinary team meetings
Complete client goal setting in consultation with patient/family/carers as appropriate. Goals are
to be functional and measurable
Evaluate treatment / management using the most appropriate methods, making use of objective
measures where possible
Achieve activity KPIs for relevant service
Maintain current knowledge and skills in clinical best practice and rehabilitation
Participate in relevant in-service programs
Supervision of undergraduate dietetics students as appropriate
Actively participate in quality improvement initiatives (includes working groups, projects, audits,
discipline related projects) as directed by Team Leader
Maintain accurate statistics and submit by end of working week
Maintain the appropriate standard of medical record keeping and data collection as per St.
Vincent’s and HIP policies
Participate in meetings as required and complete allocated tasks on time
6. INCUMBENT OBLIGATIONS
General

Maintain clinical registration and any required indemnity cover
Always work within approved scope of practice under supervision by more senior clinical staff as appropriate.
Take personal responsibility for the quality and safety of work undertaken
Take all necessary care and precautions when undertaking clinical procedures
Complete annual clinical competencies
Maintain skills and knowledge necessary to safely and skilfully undertake clinical work
Consult with peers and other experts and refer to other healthcare workers when appropriate and in a timely manner
Collaborate and clearly communicate with patients/clients and the healthcare team
Participate in clinical risk management and continuous quality improvement activities as part of day-to-day work
Ensure consumers receive information in an appropriate and accessible format
Actively support consumers to make informed decisions about their treatment and ongoing care
Ensure consumers are aware of their rights responsibilities and how to provide feedback
Protect the health and safety of self and others, complying with all health and safety related policies, procedures and directions
Complete required Fire and Emergency Training annually
Complete required Workplace Culture and Equity Training annually
Attend general hospital orientation within 3 months of commencement
As required, comply with fit-testing and PPE requirements
Participate in reporting and analysis of safety and quality data including risks or hazards,
Report any hazards, near misses and incidents (regardless of whether an injury occurred or not) into Riskman
Identify and report any variance to expected standard and minimising the risk of adverse outcomes
