Are you a passionate and experienced Disability Support Worker looking for the next challenge within this rewarding industry?

Our Mobile Rapid Response Team – Complex Care Disability Support Worker at CareChoice will offer casual or permanent part time employment opportunities and the chance to be part of a mobile team providing care throughout Victoria.

** You must be prepared to travel to and stay in regional areas when needed.**

A strong focus of this team will be utilising its field experience and expertise to provide interim support (for up to three months) to new CareChoice clients requiring urgent care and support, usually following a recent hospital admission.

This role will also provide regular opportunities to support clients based throughout regional Victoria and metropolitan Melbourne. In instances where travel and overnight stay occurs, meals and accommodation allowance support will be provided by CareChoice.

Please note - some of the pre-offer training and induction sessions may be subsidised (if eligible) as part of the recruitment and onboarding process.

A day in the life as our Complex Care Disability Support Worker, some of your key responsibilities will include:

  • Being responsible for delivering all client supports in line with the client support plan and principles of person-centred active support
  • Assisting Client Engagement Managers (CEMs) and Operations with urgent and critical support of new CareChoice clients with complex support needs

Supports include, but are not limited to:


Goal Achievement

  • Assisting clients to work towards the goals stated in their individualised support plan
  • Provide feedback to the Intake Coordinator, CEMs and Operations relating to clients’ progress towards goals
  • Complete documentation including progress notes and medication administration as it relates to client goal achievement

Activities of Daily Living

  • Support and assist clients with in-home supports and residential care supports in accordance with directions from the client, the client’s individual support plan, behavioural support plan and directions from CEMs and CareChoice Management
  • If requested in the support plan, assist with meal planning, shopping, meal preparation, budgeting, and money handling skill development. In addition, provide support and assistance with domestic tasks including cleaning, vacuuming etc.
  • Where required, assist with various complex care tasks such as hoist transfers, catheter care, bowel management, diabetes management, medication administration, PEG feeding, etc.
  • Client transportation assistance. This could be either in the Support Worker’s car, another approved vehicle, or by assisting the client to access public transport
  • Assistance to organise and/or attend appointments and engage in community activities
  • Provision of personal care support and assistance to maintain daily hygiene, with a focus on maintaining client dignity and privacy
  • Support with personal grooming and dressing
  • Clean, fitting and removal of aids and devices

General and Communication

  • Carry out instructions and follow direction from the Intake Coordinator, Team Leader members of the CareChoice Coordination Team, Support Worker Liaison Officers, other CareChoice office personnel, the client support plan, and Behaviour Support Plan
  • Adhere to CareChoice OH&S policies and maintain all OH & S workplace requirements and practices
  • Comply with the CareChoice Code of Conduct (NDIS/Staff)
  • Report any changes in the health, mental state, or condition of client to the Team Leader CareChoice Coordination Team, Support Worker Liaison Officers and/or other CareChoice office personnel
  • Report all customer complaints to the CareChoice Coordinator promptly
  • Be aware of safe working conditions and implement safe working practices in all workplaces and report all incidents and potential hazards immediately to the CareChoice office and complete incident reports immediately as required

About you


You will have a caring nature, with a willingness to demonstrate support and empowerment for people with a disability. In addition, you will have a pro-active attitude, be self-motivated, have a can-do attitude, flexibility and a willingness to respond to changing client needs.

A preparedness to work part as a team and travel to regional areas on rotational placement to meet service needs will be essential.

Ideally, you’ll have:

  • A minimum qualification of a Certificate IV or above in the following disciplines: Aged Care, Disability, Nursing. Or a recognised certificate or diploma of Community Services, or similar.
  • A minimum of 2-3 years experience working as a Disability Support Worker with complex patients.
  • Current First Aid and CPR qualifications.
  • Current Victorian Employee Working with Children Check (WWCC).
  • Current NDIS Worker Screening Check (NDIS Check).
  • A sound understanding of privacy and confidentiality requirements in client care provision.
  • A good knowledge of the issues and concerns for people with disabilities, the aged, their families and support networks.
  • A current Victorian driver licence.
  • A registered, insured, and roadworthy vehicle, and the willingness to transport clients in own vehicle.
