Nurse Practitioner Aged Care

Position Profile:

The Nurse Practitioner Aged Care provides advanced nursing assessment, diagnosis, care and intervention using innovative models to consumers, residents, patients and their families. The role functions autonomously and collaboratively in an advanced and extended clinical role with other health professionals to diagnose, treat and manage older persons health problems and uses nursing expertise and leadership that influences and progresses clinical care, policy and collaboration for aged care in WACHS and the South West position is under the professional supervision of the Regional Director of Nursing and Midwifery.
