In this role, you will conduct comprehensive health assessments, work closely with patients to understand their specific conditions and recommended treatments, and empower them to actively manage their health.
You will identify and prioritise health issues, support patients in overcoming barriers to care, and help them establish goals and develop personalised care plans. You will also coordinate referrals to appropriate health professionals and services, participate in clinical supervision, and collaborate with GP practices.

Who You Are
We’re looking for a Registered Nurse, Division 1 with current registration to practice with AHPRA, with advanced clinical skills in managing complex caseload using client-centred approaches, to join our team of service professionals.
Reporting to the Right Care = Better Health (RC=BH) Registered Nurse Clinic Lead and Team Leader Chronic Disease Management Services, your position will work alongside GP’s, practice staff to enhance team-based care. The team sits within the wider Primary Care program at EACH made up of committed health and allied health professionals aimed at ensuring patients receive “right care, at the right time, in the right place”.
